Failure Mode And Effects Analysis

FMEA (Failure Mode and Effects analysis), as a systematic group of activities, intends to recognize and evaluate the potential failure of a product/ process and effects of that failure and identify actions that could eliminate or reduce the chance of the potential failure occurring and document the entire process. All FMEAs focus on the design, whether it is of product or process.

And this reference document presents two types of FMEA: Design FMEA and Process FMEA. Any suppliers who subscribed to QS-9000 or its equivalent are allowed to use it for effective operations for a given situation. One of the most important factors for the successful implementation of an FMEA program is timeliness. It means to be “before-the-event” rather than “after-the-fact” exercises.
